Cardiovascular

Ankle swelling, palpitations, orthostatic dyspnoea, nocturnal dyspnoea, chest pain on exertion, claudication

0–1 for presence or absence of any symptom

Respiratory

Sputum production, pleuritic chest pain, haemoptysis

0–1 for presence or absence of any symptom

Alimentary

Dysphagia, indigestion, heartburn, abdominal pain, weight loss, change in bowel habit

Urogenital

Dysuria, frequency, prostatic symptoms, impairment of sexual function

Central nervous system

Headaches, fits, faints, tingling, numbness, muscle weakness, excessive thirst, abnormal sleep patterns

Endocrine

Heat intolerance, cold intolerance, change in sweating, prominence of the eyes, swelling in the neck

0–1 for presence or absence of any symptom

Visual

Eye pain, disturbance of vision

Hearing

Difficulty hearing, requiring radio or television sound turned up, difficulty detecting direction of sound, dizziness or vertigo, noises in the ears, pain in the ears
